The promoter of the Day in the Life Video Contest (the “Contest”) is Health Carousel, LLC, having its registered office at 4000 Smith Road, Suite 410, Cincinnati, OH 45209 (“Health Carousel”).  

The Contest is open to individuals who: (i) submit a video in line with the instructions on the go.passportusa.com/day-in-a-life-creative-video-contest; (ii) are legal residents of the United States of America; and (iii) have reached the age of majority, at the time of their entering this Promotion, in the state where they reside, (together, “Entrants”). All winners in this Promotion must meet these eligibility requirements to receive the prizes. There is no entry fee and no purchase necessary to enter this Contest.  

By entering this Contest, an entrant is indicating his/her agreement to be bound by these terms and conditions (the “Terms”). 

Recitals

1. Only one entry will be accepted per person. Multiple entries from the same person will be disqualified.  
2. Entries may be submitted beginning on August 26, 2024 and ending on October 31, 2024 at 11:59 PM (EST). After this date no further entries to the Contest will be permitted. 
3. Health Carousel shall have no responsibility for entries not received, for whatever reason. 
4. Health Carousel reserves the right to cancel or amend the Contest and these Terms without notice in the event of a catastrophe, war, civil or military disturbance, act of God or any actual or anticipated breach of any applicable law or regulation or any other event outside of Health Carousel’s control. 
5. Health Carousel is not responsible for inaccurate prize details supplied to any entrant by any third party connected with this Contest. In the event of a conflict between any information supplied by a third party and the information contained in these Terms, these Terms shall govern.  
6. Three (3) prizes will be awarded in this Contest. Odds of winning depend on the number of Entrants. Each prize will consist of: 
   
   One (1) Amazon gift card, valued at $250 per winner.
   
   The prize is as stated, and no cash or other alternatives will be offered. The prize is not transferable. Prizes are subject to availability, and Health Carousel reserves the right to substitute any prize with another of equivalent value without giving notice. 
   
7. Winners will be chosen by Health Carousel judges, from all entries received and verified by Health Carousel and/or its agents. Each entry will be judged based on creativity, relevance to the category, impact, quality and storytelling, in Health Carousel’s sole discretion. 
8. The winner will be notified by email within five (5) business days of the closing date. If the winner cannot be contacted or does not claim the prize within fourteen (14) calendar days of notification, Health Carousel reserves the right to withdraw the prize from the winner and pick a replacement winner. 
9. Health Carousel will notify the winner when and where the prize can be collected and/or collect information pertaining to where the prize should be delivered. Health Carousel shall not be liable for the winner’s failure to collect and/or provide the appropriate information for delivery or for any delivery issues that may arise as a result of the carrier’s negligence or intentional misconduct.  
10. Health Carousel’s decision in respect of all matters to do with the Contest will be final and no correspondence will be entered into. 
11. By entering this Contest, an Entrant is indicating his/her agreement to be bound by these Terms. 
12. The Contest and these Terms will be governed by Ohio law and any disputes will be subject to the exclusive jurisdiction of the courts of Hamilton County, Ohio.  
13. By entering this Contest, an Entrant is consenting to Health Carousel’s utilization of the Entrant’s submitted photo or video, name, likeness, appearance, voice or other personal characteristics, in any medium or format, in connection with the marketing, sales, educational, promotional and/or other endeavors of Health Carousel. In addition, Entrant agrees to allow Health Carousel to digitize, modify, alter, edit or adapt any image, video or recording as necessary. 
14. This Contest is in no way sponsored, endorsed or administered by any company outside of Health Carousel. Entrants are providing information to Health Carousel and not to any other party.
